Short-term rehabilitation - Our dynamic rehabilitation department offers Physical Therapy, Occupational Therapy and Speech Pathology services in our state-of-the-art therapy suite equip with kitchen and bathroom. Programs are established to meet the specific needs of each resident. Our aggressive short-term rehabilitation regimen is designed to help residents return home as soon as possible. Some of our therapy services include: assessment and management of equipment needs, splinting and orthotics, fall prevention programs, therapeutic exercises (strengthening and conditioning), functional mobility (ambulation, gait, balance, wheelchair mobility), fine motor muscle re-education, cognitive and perceptual retraining, language and communication retraining, community re-entry training.
Long-term care - For residents who have medical requirements including Dementia and Alzheimer’s care we provide 24-hour nursing care, nutritional monitoring and planning, medication administration, personal care and other medical services such as on-site psychiatry, podiatry, dentistry, audiology and ophthalmology.
Respite care - This program is designed to assist family members who may need a short-term break from their ongoing care giving duties. Respite care can also provide the necessary short-term assistance during a personal or medical crisis.
Cardiopulmonary rehabilitation - Is designed to improve the health and quality of life of patients with heart, lung and vascular disease.
Palliative/Hospice care - Any form of medical care or treatment that concentrates on reducing the severity of disease symptoms, rather than trying to halt, delay, or reverse progression of the disease itself or provide a cure. The goal is to prevent and relieve suffering and to improve quality of life for people facing serious and complex illness. Connecticut Hospice assists us with providing these services through their Home Care Program. The hospice-based philosophy is applied throughout the resident’s stay.
